In accordance with the new Data Protection guidelines all companies need to offer clients and customers transparency and ease of access to their own data.

What data is kept:

New and Returning clients complete a Health Questionnaire. This questionnaire has client name address and contact details, general lifestyle and questions regarding health and physical/mental issues.

How it gets kept:

As owner of The Movement Company, I Gemma Carpenter take responsibility for storing client information. Questionnaires are kept in paper format as completed by the client and stored in a folder – some information gets copied on to a password-protected spreadsheet on password-protected computer.

Why it’s kept:

Name and contact (phone/email/address/emergency contact) details – Seen by The Movement Company ONLY for contacting clients about lessons, sessions and all else relating to The Movement Company business.
Jobs/Hobbies/age/how client heard about lessons - Seen by The Movement Company ONLY for compiling demographic information
Health/Physical/Mental issues – Seen by The Movement Company ONLY to ensure needs are met during sessions.

How long it gets kept for:

All data is stored for active clients. Data is kept for no longer than 6 months after a client leaves. Paper data is destroyed on disposal. Electronic data is deleted.

Any client can request access/amendments/deletion of their own records in writing at any time and The Movement Company will endeavor to complete their request within the quickest time possible.

Any breach of data shall be reported to the client as soon as possible. Any serious breach of data will be reported to the local authority.

All Health Questionnaires ask for client permission to take photos. Request shall also be asked for at time of taking/publishing photos as well - consent can be removed at any time

Health questionnaires also request permission for The Movement Company ONLY to contact the client using the details provided by the client.
